SSC Covid-19 Self Declaration format 2020 is now available in pdf format at Official website ssc.nic.in.
Required Details for SSC Corona virus Self Declaration form
The Candidates need to submit the Covid-19 form before the Last date. The Candidates will have to provide Name , Roll no., Exam Name, Exam Date, Exam Shift, Exam center Name and Signature.
